In care work, human presence is not delivering something a machine could deliver more efficiently. Human presence is the product.

The internal test for each item: Would a human or machine doing this instead produce a categorically inferior outcome — not merely a less efficient one?
The care pair is the starkest in the series. In every preceding domain — education, finance, construction, healthcare, law, governance, science — the human column produces something that AI cannot replicate: relational attunement, moral judgment, deliberative legitimacy. But in care work, the relationship between the columns is not merely complementary. It is asymmetric in a specific way: the human column is not a means to an outcome. It is the outcome. A person who is genuinely witnessed, known, and cared for by another person has received the product. There is no efficiency gain that substitution could produce because there is nothing to optimize — the presence itself is what is being delivered.
Bowlby (1969), in Attachment and Loss, established the foundational framework: human attachment is not a preference or a comfort. It is a developmental necessity. Infants who do not form secure attachment bonds with caregivers develop measurably different neurological, emotional, and social architectures. The attachment system is not a feature of human psychology that could be satisfied by a sufficiently sophisticated substitute. It is a biological mechanism calibrated to detect and respond to the presence of another human being.
Rutter (1998) provided the most devastating natural experiment: the Romanian orphan studies. Children raised in institutions with adequate nutrition, shelter, and physical care but severely limited human relational contact showed profound developmental impairments — cognitive, emotional, social, and neurological — that persisted years after adoption into caring families. The deprivation was not material. The children had food, warmth, and safety. What they lacked was human relational presence. The documented consequences confirm that human presence in care is not a delivery mechanism for an outcome that could be delivered otherwise. It is the outcome.
Winnicott (1965) introduced the concept of "good enough" caregiving — the recognition that what children need is not perfect care but consistent, attuned, human care. The "good enough" standard is important because it establishes that the human column does not require exceptional performance. It requires presence. A caregiver who is consistently available, emotionally attuned, and responsive to the child's states — even imperfectly — provides what the child's developmental system requires. No machine, however technically perfect, satisfies this requirement because the requirement is for a human relationship, not for the functions that a human relationship performs.
The right column of the Pair table represents capabilities where AI and robotics can genuinely improve care outcomes: medication management at scale, continuous health monitoring without fatigue, fall detection and emergency response, physical assistance exceeding human endurance, and the administrative burden — documentation, billing, coordination — that currently consumes a substantial portion of caregiver time and energy.
The AARP Public Policy Institute (2023) documents 53 million unpaid caregivers in the United States. These caregivers provide an estimated $600 billion in unpaid labor annually. They experience measurably elevated rates of depression, physical health decline, financial hardship, and social isolation. The caregiver burden is not primarily relational — caregivers generally find the relational aspects of caregiving meaningful. The burden is administrative, physical, and logistical: managing medications, coordinating with healthcare systems, providing physical assistance beyond their endurance, and handling documentation requirements.
An FTP-compliant care design would deploy AI and robotics in the right column — handling the administrative, physical, and logistical burden that exhausts caregivers — freeing human presence for the left column: being with the person, knowing them across time, providing the relational presence that constitutes care.
Wada et al. (2008) documented that PARO, a robotic therapeutic seal, produces measurable stress reduction in dementia patients. This finding does not undermine the argument. It clarifies it. PARO is FTP-compliant when it supplements human care — providing sensory engagement during the hours when human caregivers cannot be present. Every caregiver sleeps. Every caregiver has other responsibilities. During those gaps, a device that provides sensory stimulation, reduces agitation, and offers tactile comfort serves a genuine function.
PARO fails FTP when it substitutes for human care as a cost-reduction measure. Sharkey & Sharkey (2012) drew this distinction precisely: the ethical concern with care robots is not their existence but their deployment logic. A care robot deployed to fill gaps in human availability is supplementary. A care robot deployed to reduce the number of human caregivers required is substitutive. The first preserves the human column while supporting it with the machine column. The second erodes the human column to reduce costs.
The distinction is not between good technology and bad technology. It is between technology deployed to free human presence and technology deployed to replace it.
The current trajectory in care technology is overwhelmingly substitutive. Investor interest concentrates on technologies that reduce labor costs — robotic caregivers, AI companions, automated monitoring systems designed to reduce the human staff required. The supplementary design — technology that handles administration, logistics, and physical assistance so that human caregivers can be more present, not less present — receives a fraction of the investment because it does not reduce headcount. It improves care quality, which the market values less than cost reduction.
Fidelity: Fails under substitutive deployment. The dominant design trajectory deploys care technology to reduce human caregiver requirements, not to free human caregivers for relational work. The 30-day test is stark: if AI care systems were removed, would care recipients have adequate human relational presence? In many institutional settings, the answer is already no — not because AI caused the deficit, but because the deficit is the market condition that AI is being deployed to accommodate rather than remedy.
Transparency: Partial. Care robots and monitoring systems generally disclose their functional purpose (Level 1). Most do not disclose their data collection practices, algorithmic decision-making processes, or the commercial interests that shape their deployment (Level 2–3). Care recipients — often elderly, cognitively impaired, or very young — cannot evaluate these disclosures even when provided.
Participation: Fails. Care recipients have no structured governance input into the design or deployment of care technologies. Deployment decisions are made by institutions, families, and vendors. The populations most affected — the elderly, people with dementia, young children, people with disabilities — are precisely the populations least able to advocate for their own relational needs in technology design processes. The consent deficit is structural, not incidental.
The documented consequences of substitutive care technology deployment are predicted by the evidence base already established. Rutter (1998) documented what happens when human relational presence is inadequate during development: measurable, persistent impairment across cognitive, emotional, social, and neurological domains. Cacioppo & Patrick (2008) documented what happens when human relational presence is inadequate in adulthood: dose-response health consequences equivalent to established risk factors for morbidity and mortality.
The stakes in care are compounded by a specific structural feature: the populations most affected by care technology deployment are the populations least able to advocate for themselves. Infants cannot articulate their need for attachment. People with dementia cannot evaluate whether a robotic companion substitutes for or supplements human relationship. Elderly people in institutional care often cannot choose their care arrangements. The consent deficit is not a solvable design problem — it is a permanent feature of care contexts that requires external governance to protect relational needs that care recipients cannot articulate or advocate for themselves.
The 53 million unpaid caregivers documented by AARP (2023) represent both the scale of the care burden and the population whose relational labor is most at risk of substitution. If care technology reduces the perceived need for human caregivers — by providing monitoring, companionship simulation, and physical assistance — the social infrastructure that supports human caregiving (family leave policies, caregiver support programs, respite care funding) will face reduced political support. The substitutive design does not merely replace individual caregivers. It erodes the institutional conditions that make human caregiving sustainable.
The care pair completes Series 1 of The Capability Pairs. Across eight domains — education, finance, construction, healthcare, law, governance, science, and care — the three-axis analysis has identified the irreducible human column, the irreplaceable machine column, and the stakes of getting the deployment design wrong. Each domain produced a named condition identifying the specific structural feature that makes the human column irreducible in that context.
The care pair provides the starkest formulation: human presence is not a means to an end in care. It is the end. The Non-Simulable Relationship is not a limitation of current technology that future technology might overcome. It is a structural feature of human biology, developmental psychology, and the relational architecture that sustains health across the lifespan. Care technology that supplements human presence serves the human column. Care technology that substitutes for it does not merely fail to provide care — it produces the documented health consequences of relational deprivation while creating the institutional illusion that care is being delivered.
